They fall under the broader group of pelvic insufficiency fractures. Sacral insufficiency fractures are a frequent cause of both acute and chronic pain; however, they are often missed by the majority of physicians. The frequency of undetected sacral fractures is high.

Insufficiency fracture sacrum

Findings on plain films and conventional tomograms are often subtle. Sacral insufficiency fractures (SIF), as the name refers, are a type of fracture involving the sacrum. They were first described as recently as 1982 by a physician named Lourie. SIF fractures are unique and can occur when the sacral bones are no longer dense enough to handle the stress of weight bearing. Sacral stress fractures are of two types: (1) Insufficiency fractures are stress fractures of the sacrum resulting from normal stress on abnormal or demineralized bone and (2) fatigue fractures are stress fractures that occur when abnormal or demineralized bone is subjected to abnormal stress.

These fractures, which are due to osteopenia, form a distinct subgroup of pathologic fractures.
